We accept
assignment on many insurance plans, however, it is the patient's
responsibility to provide us with up-to-date insurance cards. Some
insurance companies require precertification for certain exams. The
patient should be aware of this prior to making an appointment. If
a precertification number is required, the referring physician must
get the procedure authorized and either provide our office or the
patient with the authorization number. We make every effort to determine
if precertification is needed, however, insurance requirements change
continually and every insurance plan is different, so we strongly
urge the patient to be aware of what their insurance company requires.
Below is a list of insurance plans which we accept and their precertification
requirements. Please call our office if you do not find your insurance
on this list.
All insurance companies allow one screening mammography per
year. Medicare allows pelvic sonograms every six months. Again,
these rules are all subject to change. Please check when making
your appointment.
